Partnership HealthPlan of California is sponsoring a one-day training for participants to learn the importance of and how to guide discussions about Advance Care Planning.

This FREE training is designed for providers, nurses, social workers, and other primary care practice staff who discuss advance care planning with patients. Six CEUs available for nurses and social workers. The training is limited to 40 participants.

Faculty from the Coalition for Compassionate Care of California will be leading this one-day training:

Course Objectives:

  • Understand the evidence base regarding the importance of advance care planning conversations.
  • Recognize common barriers and misconceptions associated with advance care planning.
  • Identify strategies for addressing barriers to engaging in advance care planning conversations.
  • Practice communication skills for conducting culturally-congruent advance care planning conversations across the life-span.
  • Have increased confidence in supporting the completion of advance healthcare directives.
  • Increase your familiarity with a range of practical tools and resources to assist you in the integration of advance care planning conversations in your setting.
